Accidents Happen

Injury and non-injury accidents can cause job completion delays and can increase the costs of whatever operation is underway. Most, if not all, accidents can and should be avoided. However, in the real-world accidents do occur. So, what do you do when an accident does happen?  Accident investigations are crucial in determining what caused an accident (injury or non-injury) and how to avoid them in the future, reducing chilling injuries, the time and money spent on medical treatment and bills, time lost on the job, and the headache of dealing with all the above.

Accident Investigations

First and foremost, the purpose of an accident investigation is not to assign blame to any one person or thing.  The purpose of an accident investigation is to find the root and all other contributing factors of the accident and creating a solution to address the issue.  An accident investigation needs to be conducted as soon as possible.  If an injury resulted from the accident, make sure to address all immediate needs of the victim. So long as no life threatening or other serious medical concerns are present the investigation needs to commence as soon as possible.  Witnesses need to be interviewed quickly since human memory is not particularly strong days or even hours after an incident.  Pictures of the area and documentation of the work site as it was at the time of the accident are crucial in determining all factors that may have contributed.  DO NOT tamper with or clean up the work site area after an accident and before the investigators have documented all that they need.  Again, the purpose is not to assign blame just to determine the causes and rectify the problems to stop future incidents.  Accident investigations should be conducted by knowledgeable individual(s) trained in conducting accident investigations and should be done whenever damage to a person or equipment happens, or when one of those two things nearly happens also known as a near miss.  Any fatality, amputation, eye loss, or in-patient hospitalization will be reported to OSHA within 24 hours.
